How It Works

The Health-e-Connect (HeC) System is composed of the following:

  • HeC Remote Care System – a web based application for Health Care Providers to improve compliance and adherence to care plans of patients in their homes.
  • Any current HeC compatible medical monitoring device.
  • A mobile phone.

The following diagram illustrates the system at work:

HEC Process Flow

  1. Health Care Provider enrolls patient into the HeC System and configures system monitoring parameters.
  2. Patient follows prescribed health maintenance program activities and monitored data is collected.
  3. At specific intervals, data is uploaded to the ALRT server.
  4. The HeC System validates the uploaded data versus the Health Care Provider programmed limits. When out-of-limits data is encountered, the system alerts healthcare provider.
  5. Patient’s primary healthcare provider, the patient, secondary healthcare provider and patient’s family members may send messages to each other whenever necessary.
  6. All parties with authorized access may login to HeC anytime to view the accumulated data from a patients health maintenance activities.

Steps 2 through 4 is repeated.
